Essential Duties and Responsibilities: Perform initial and ongoing assessments of resident physical function and mobility.Develop, implement, and revise individualized treatment plans based on assessment results.Administer physical therapy treatments and interventions in line with best practices.Educate residents, families, and staff on proper techniques and home exercise programs.Document evaluations, treatment sessions, progress notes, and discharge summaries accurately and timely.Collaborate with the interdisciplinary care team, attending care conferences and assisting with discharge planning.Supervise and delegate duties to Physical Therapist Assistants (PTAs) as required by Colorado licensure laws.Ensure compliance with Medicare, Medicaid, and facility documentation and billing standards.Maintain equipment and work areas to ensure safety and cleanliness.
